  1. Gather your documents: proof of residence in a covered ZIP code, medical diagnosis, and ID.
  2. If applying on behalf of someone who has passed, include the death certificate and legal authority (will, trust, or authorization).
  3. Download the official DOJ RECA application below.
  4. Print it out and complete it carefully. Keep copies of everything for your records.
